UpHill’s clinical pathways are interactive and dynamic representations of protocols or guidelines based on the best scientific evidence. They serve as a clinical decision support tool for healthcare professionals at each critical point of a specific disease or symptom.
Actions
Throughout a pathway, actions consist of recommendations at specific points in the clinical journey.
Regarding the different versions of the algorithms, unlike the patient pathway version, the "Stand Alone" version is read-only, meaning it is not possible to select actions or the items that compose them.
Each action has multiple selection options, left to the healthcare professional’s discretion, such as:
The action as a whole
If there are no items, all items composing it are automatically selected.
The date and time of the action are recorded.
Single item
If the item is circular, only one option can be chosen.
The date and time of the action are recorded.
Multiple items
If the item is square-shaped, it is recommended to select all available options. However, the professional may select one or several options, based on clinical judgment.
Actions are grouped by categories, each associated with a different color.
Action Statuses
Statuses associated with actions and/or items indicate progress, allowing detailed tracking of each task. They can be updated throughout the pathway as states change.
Different action categories may have different statuses, all representing whether the action is completed, in progress, or canceled/not performed:
Complete: green
In Progress: orange
Canceled: gray dash
Items within actions can also have associated statuses.
NOTE: All statuses should be updated by the healthcare professional according to progress, ensuring chronological documentation.
Actions may include multiple items, each of which can also have an individual status. Depending on how the professional sets the status:
Setting a status for the action automatically applies it to all items.
Setting a status per item marks the overall action complete only when all items share the same status.
If at least one item is incomplete, the overall action remains “in progress.”
For certain statuses, the professional may be prompted to provide a reason. This is optional and multiple selections are allowed, or they can go back and change selections. Mistaken status selections can be undone within the first “X” hours.
Examples of Actions
Some actions contain detailed information displayed on the right when clicked, including:
Recommendations
Additional details
Supporting references
These details may appear as:

PDFs
Actions without details display: "This action has no associated details."
Specialties – indicate referrals to specific specialties.
Interventions – triggered when the patient’s state indicates intervention is needed.
Prescriptions – recommend specific medications.
Examinations – indicate when diagnostic tests are appropriate.
Gateways
Gateways are complex elements that influence the patient pathway based on the professional’s decisions. The number of selectable options is shown in a blue square at the top-right of the gateway.
Types of Gateways
Exclusive Gateway: Only one option may be selected (as indicated by the blue number).
Parallel Gateway: All options must be completed for the algorithm to proceed. Once all paths are completed, a message appears.
Inclusive Gateway: One or more options may be selected, depending on applicability. Each option leads to a sub-path, but all converge back to the main algorithm path. Messages appear after completing paths with options to Explore or Continue.
Rules Gateway: One, several, or all options may be selected. The resulting path may vary according to selections. Clicking an option automatically shows the next step.
Medical Calculators
Medical calculators are evidence-based formulas integrated into the algorithms. Results may lead to different paths. Unlike actions and gateways filled within the algorithm, calculators are completed in the details bar but their outcomes appear in the algorithm.
Events
Events represent and record the patient’s state and can occur at:
Initial: upon entry
Intermediate: during the pathway
Final: all algorithms conclude with the final patient state
Pathway Links
Pathway links connect algorithms. At the end of a pathway, a professional may be redirected to another algorithm at a specific location.
